For side sleepers whose hip pain keeps waking them

why hip pain can keep waking you up, even when you're tired enough to sleep

I flip from side to side all night like a rotisserie chicken, and I kept blaming the bed. Then one question at a specialist appointment changed where I looked for the problem.

- By Donna Sharlton

It's 1:40 in the morning and I'm awake again.

I'm 58, and I've been dreading my nights for the past few years.

I drift off on my left side. Then the ache on the outside of my hip pulls me back up.

So I do the routine. Roll onto the right side. Move the pillow between my knees. Pull the covers back up.

My husband hasn't moved. I stare at the ceiling fan, waiting for the ache to settle enough to fall asleep again.

By morning, I can remember turning over more clearly than I can remember sleeping.

During the day, I could get on with things. There were moments when I barely noticed my hip. But lying on that side made it hard to ignore.

So I decided the problem was the bed.

Everything I bought for the night

I'm a side sleeper. I have been all my life. Sleeping on my back never felt natural.

Four mattresses in six years, a topper in between, the last one firmer and more expensive. That's what the salesperson said. “Firm was better for hips.”

After a while, the firmer one felt like the worst choice of all. More bedding to try. More money spent. I stopped counting the pillows.

The knee pillow helped quite a bit. It really did. But I could still wake with the same ache.

I tried stretching. Reached for a painkiller on a difficult night.

Eventually, I took it to my doctor and asked whether menopause had something to do with it.

The answer came back to age, arthritis and trying to sleep on my back. I was describing nights broken by pain.

I left feeling as though the problem was the way I slept.

I still didn't know why that one spot kept waking me. And sleeping another way didn't make it any easier to do.

The question that changed where I looked

Later, at a sports-medicine appointment, I started with the mattresses. Then came a simpler question she asked:

Where, exactly, did it hurt?

I put a finger on the outside of my hip. The spot I kept lying on.

She told me that pain there doesn't always come from the hip joint.

Tendons attach the buttock muscles to the outside of the hip. When those tendons become painful, one possibility is a condition called gluteal tendinopathy.

The location alone doesn't give you a diagnosis, but it gives your doctor something to examine, and it gave me something better to do than choose the next mattress.

The position is familiar. The hip underneath it can change.

I've slept on my side all my life. That doesn't mean the tissue I'm lying on has stayed the same.

Estrogen plays a role in keeping tendon tissue healthy, but that support drops off around menopause.

So, as outer-hip tendon pain shows up around menopause, it often gets brushed off as “getting older.”

If it started around the same time as everything else changed, that's worth telling your doctor instead of brushing off.

Side-lying puts real pressure on the outer-hip tendons, and letting the upper knee fall across the body adds even more.

Tight muscles around the hip add to that pressure too, so there's often more going on than which side you sleep on.

That helps explain the pillow. Supporting the upper leg can reduce pressure. But support alone may not get you through another night.

A different surface can change the pressure. It doesn't tell me why the tissue around my hip has become sore.

If your pain sits in the same place, ask your doctor or physiotherapist whether gluteal tendinopathy could be involved.

Before buying another bed, ask about the hip that keeps waking you.

The Broken-Night Loop starts before 1:40

The ache doesn't always wait for bedtime. Sometimes it's already there at 10 p.m., while you're still on the couch.

Then you lie on that side, wake at 1:40 and start turning over.

After a restless night, the next day feels harder before it has properly started. Less energy. Less patience. A familiar task you put off because you're exhausted.

There is a physical connection too.

In sleep studies, people feel pain more after a bad night.

So the night doesn't just interrupt sleep, it changes how the hip feels the next day.

I started calling it the Broken-Night Loop.

The hip is sore before bed.

The ache interrupts sleep.

Poor sleep can make pain feel sharper the next day.

By the next evening, you're tired and trying to get comfortable again.

At 1:40, moving the pillow is what you can do. But that doesn't have to be the only time you do something for the ache.

Before the ache reaches your pillow, it's already in the muscle.

Lying on that hip for hours slows blood flow to the area and lets the muscles around it tighten, the same tight muscles that were already pressing on the tendon before you got into bed.

That's the ache waiting for you at 10 p.m., and it's still there at 1:40 in the morning.

A pillow moves the leg. A mattress changes what's underneath you. Neither one reaches the muscle that's actually tight.

That's why I finally stopped shopping for the bed, and started looking for something that helps with the muscle itself.

What I found was an evening routine built for exactly that, the outer hip, the muscle around it, twenty minutes before the turning starts. That's Halsten Stride.

Work on that muscle before 10 p.m., and there may be less of it to lie on by 1:40 in the morning.

Questions

Can I wear it while I sleep?

No. This is a 20-minute session while awake. Remove the device before going to sleep.

Should I keep using my knee pillow?

Keep it if it helps. Stride fits alongside the support and care you already use.

Will this fix what's wrong with my hip?

Stride is for temporary relief, not a repair for an injured tendon or a treatment for gluteal tendinopathy, arthritis or a sleep disorder. A more comfortable night is the goal, not a guaranteed result. If pain is new, severe or getting worse, speak to an expert.

Can I use it alongside my current care?

Check the device's suitability guidance with your clinician, especially if you take medication or have a medical condition. Keep following prescribed care.

I was told it is bursitis. Does this mean that diagnosis was wrong?

Not necessarily. Several problems can cause pain around the hip. Ask your doctor or physiotherapist about the location and nighttime pattern rather than changing a diagnosis yourself.

How long before I notice a difference?

Responses vary. With consistent use, pay attention to how you settle, how often the ache wakes you and how you feel in the morning. You have up to 90 days from delivery to decide whether to keep it.

What if it isn't for me?

Email info@tryhalsten.com before day 90 from delivery to arrange a return. Return it in good condition and its original packaging within the 90-day window for a full refund. You cover return shipping.
